BREATHE
Chronic Obstructive Pulmonary Disease, (COPD) is a progressive and irreversible condition.

Breathe was developed by Hudson Valley Hospice in collaboration with one of the region’s leading pulmonologists to provide specialized, compassionate care at home for patients living with end-stage COPD and other advanced respiratory diseases, including pulmonary fibrosis, interstitial lung disease and bronchiectasis.

HOW DOES BREATHE WORK?
- An individualized plan of care is created in collaboration with physicians to address symptoms, goals and personal needs.
- Our clinical team is available 24 hours a day, 7 days a week to provide guidance, support and assistance when urgent needs arise.
- Regularly scheduled visits from specially trained hospice professionals focus on comfort, symptom management and quality of life.
- Respiratory therapies and supportive services are tailored to ease breathing difficulties and enhance daily living.
- Specialized respiratory equipment, medications and oxygen therapy are provided as recommended by physicians.
- Access to the full range of Hudson Valley Hospice services, including Music Therapy, volunteer support, home health aides and emotional and spiritual care for both patients and families.

HOW DOES BREATHE WORK?
- An individualized plan of care is developed with your doctor to manage your symptoms and meet your needs.
- You have access to our Registered Nursing Staff 24/7 for help with handling emergencies.
- You have regularly scheduled home visits from specially trained medical staff.
- You will receive respiratory therapies and services that address comfort and quality of life.
- Specialized respiratory equipment, medications, and oxygen will be provided as recommended by your doctor.
- You will have access to all of Hudson Valley Hospice’s services, including Music Therapy, volunteer services, home health aides, and spiritual and emotional support.
