Our team helps improve quality of life for people living with advanced cardiac disease by reducing hospital visits and unnecessary medical crises through compassionate symptom management and supportive care provided wherever home may be, in a private residence, nursing home, assisted living community, group home or hospital.
- An individualized plan of care is created in collaboration with physicians to address symptoms, goals, and personal needs.
- Our clinical team is available 24 hours a day, 7 days a week to provide guidance, support and assistance when urgent needs arise.
- Regularly scheduled visits from specially trained hospice professionals focus on comfort, symptom management and quality of life.
- Specialized medications in the home, as well as specialized respiratory equipment, medications and oxygen therapy, if needed.
- Access to the full range of Hudson Valley Hospice services, including Music Therapy, volunteer support, home health aides, and emotional and spiritual care for both patients and families.


- Your symptoms are managed through an individualized plan of care, with the participation of your doctor.
- You have regularly scheduled home visits from a specially trained cardiac nurse, as well as access to our 24/7 on-call emergency staff.
- You have specialized medications in your home, as well as any necessary equipment.
- You may access other services such as music therapy, and spiritual and emotional support services.
- If necessary, a respiratory therapist is available to help you with specialized equipment.

Heart-to-Heart services are covered by Hospice Medicare, Medicaid, Veteran’s Benefits, and most insurance plans. Financial assistance may also be available.
