Hudson Valley Hospice offers a wide range of services for the patient and for the patient's family, friends, and loved ones. We offer individualized care for patients with cancer, heart disease, Alzheimer's, or respiratory disease. We provide special care for veterans through the We Honor Veterans program. We have provided education and awareness on end-of-life issues in the Dutchess and Ulster county region for more than 30 years.
Hospice care can be provided at home, or in a nursing home or other facility. Hospice services are covered by Medicare, Medicaid, Veterans Benefits, and most insurance plans. Financial assistance may also be available. Our specialized interdisciplinary team consists of nurses, social workers, chaplains, and volunteers, all trained to help you.
Getting started is sometimes the hardest part. Too often, referrals for hospice care are not made until the patient has uncontrolled symptoms or unmanageable pain. Many hospice patients and their families wish they had called Hudson Valley Hospice sooner: The patient can benefit much more from hospice services if referred early, and the family can be better prepared and more able to manage the care process.
Remember that anyone can contact hospice directly. Hudson Valley Hospice will contact the patient's doctor to discuss if the patient is medically appropriate for hospice care. If you think Hudson Valley Hospice can help you, a family member or friend, please call us at 845-485-2273. We're here to listen, to comfort and to serve.
When a patient reaches the end stages of an illness or disease, hospice offers palliative care to help control pain and reduce anxiety. We improve quality of life and time spent with loved ones.
Emotional and Spiritual Support
Hospice care provides emotional and spiritual support to patients and families, including grief and bereavement resources. You and your loved ones don’t have to face these challenges alone.
Medical Care Management
Hospice manages medical care, in-home visits from care providers, acquiring medical supplies, and end-of-life planning.
Respect for the Patient and Family Wishes
Hospice strives to provide the highest quality end-of-life care so that our patients may live with comfort and dignity. Every family is on a different journey, and we acknowledge and respect your decisions and plans. Our goal is to make this difficult time as easy as possible.
We improve quality of life by relieving patients and their loved ones of much of the stress that comes with providing end-of-life care. Families can focus on being together and sharing memories.
